2.Plan Levels: Modular Hospitalization

The ACS Expat plan offers four levels of hospitalization cover. The core medical procedures (surgery, ICU, and oncology) are covered at 100% across all plans, but the following differences help you choose the right fit:

  • Bronze: The most budget-friendly ‘inpatient only’ option. It offers an annual limit of €500,000 and covers a semi-private hospital room. This is ideal for those seeking essential protection at the lowest premium.
  • Silver: Increases the safety net with an annual limit of €1,000,000. It includes coverage for a private hospital room (up to a daily limit) and adds coverage for psychiatric hospitalisation. 
  • Gold: A comprehensive mid-range choice with a €1,500,000 annual limit. It provides full coverage for a private hospital room and is the first level that allows you to add the Maternity module.
  • Platinum: The highest level of security with a €2,000,000 annual limit. Like the Gold plan, it offers a full private room and additional maternity options, but with the highest possible ceilings for all medical interventions.

3.Optional Add-ons

Tailor your policy by adding one or more of the following modules to your hospitalization base:

  • 3.1 Routine Medical Care (Outpatient): Covers GP visits, specialists, prescribed medication, and diagnostic tests.
  • 3.2 Optical & Dental: Covers routine dental care and corrective lenses. Requires the Routine Medical Care module.
  • 3.3 Maternity: Coverage for childbirth and prenatal care, but: max reimbursements also for complications. ACS does not offer the best maternity cover available.  Available only for Gold and Platinum plans.
  • 3.4 Assistance Plus: Includes repatriation of the body and other useful covers. At about 8 euro per person per month this is a very useful addition to your healthcare insurance, so much that we truly recommend you issue this add-on along with your healthcare insurance.  

4.Premiums

ACS premiums are designed to be transparent and fair. They are based on your age, the chosen level of coverage, deductible and your destination.

  • No hidden costs: There are no surcharges for monthly, quarterly, or semi-annual payments.
  • Stability: Premiums are community-rated, meaning they remain more stable over time as you age compared to many other providers.
  • You can choose for a deductible of 500, 1000, 5000 of 10.000 euro per year to lower your premium
  • There is just a small difference in premium between Silver and Platinum so choose your cover wisely. Platinum offers much higher reimbursements, for example 100% costs of private room versus max 150 euro per day at Silver cover
  • Premium for cover ‘Assistance Plus’ is approximately 8 euro per person per month
  • Fixed additional annual fee of 20 euro per policy applies

ACS does not provide premium tables or an online quotation tool. There are a two ways to receive a personalised quotation:

  1. Immediate insight: Use our online tool to calculate premiums This tool contains more insurers and provides a quick overview of premiums and covers. You cannot apply for the insurance via this tool, for instructions on how to apply see point 7 below. 
  2. Wait max 2 to 3 working days: Ask us to send you a quote by email. To do so please send us (contact@johoinsurances.org) an email with the following information:
    1. Names and dates of birth of all persons that need to be insured
    2. Country of nationality
    3. Country of stay (minimum 1 year)
    4. level of required cover (Bronze, Silver, Gold or Platinum)
    5. Inpatient only? Or including outpatientcover? 
    6. Any additional cover? (optical and dental / maternity / Assistance Plus) When in doubt please check the Table of Benefits in the brochure
    7. Add a deductible? (choose between 500, 1000, 5000 of 10.000 euro per person per year)
    8. Send the email to contact@johoinsurances and mention in the subject ‘request for quote ACS Expat + your name’ 
    9. We will send you a quote per email within 2 to 3 working days

5.Important remarks

  • Add ‘Assistance Plus’ cover to be insured for (for example):
    • Transport of the body if an insured person dies (actual costs)
    • Shipping of medicine if locally not available (actual costs)
    • Compassionate travel (multiple covered situations - see TOB)
  • Waiting periods apply - see point 6 below
  • If the Member opts for a higher level of cover, a new medical acceptance procedure will be required and new waiting periods will apply. 
  • Returning to a lower level of cover than the level previously chosen will give rise to a two-year period during which the Member may not opt for higher levels of cover. 
  • A change of deductible can only be made once during the life of the policy, at the date when enrollment is renewed. If the deductible is reduced or removed, a new medical questionnaire may be required.
  • If you move to live in another country, the geographical zone of your new country of expatriation determines whether you can continue your insurance without new medical approval. 
    • If you move within your current geographical zone or lower then no new medical acceptance procedure is needed. 
    • If your new country of stay is in a higher zone a new medical acceptance procedure is required. 
    • ACS uses three geographical zones; see the policy conditions at "territorial scope of cover" for an explanation. 
    • The premium in a new country of stay may be higher or lower, depending on the premium ACS applies for that specific country, even if the new country is part of the same geographical zone of your former country of stay. Ask us to help you move from 1 country to another and we will guide you along the way. 
  • Newborns can be covered from birth without a health questionnaire (except with multiple births or the adoption of a child placed in a care centre or foster family), provided ACS is informed within 30 days of the child's date of birth and the birth certificate issued by the hospital is sent. If ACS is informed of the addition of a newborn more than 30 days after the birth, the Member must complete a health questionnaire for this child and wait for the Insurer's approval. 

6.Waiting Periods

To keep premiums competitive, the following waiting periods apply:

  • Dental care and psychiatric hospitalisation: 3 months.
  • Dental prostheses - prescription lenses and frames - laser treatment of visual corrections (including related surgical treatments): 6 months.
  • Orthodontics: 9 months.
  • Maternity: 12 months.
  • Medically assisted reproduction: 18 months.

However, expenses incurred following an Accident are reimbursable immediately.

7.How to Apply?

  1. Application Form: Fill out the Application Form for you (and your familymembers)
  2. Medical Questionnaire: Complete a brief health declaration. Use 1 questionnaire per insured family member. Example: if you have a family of 4 and you want to insure everybody you need to fill out 4 medical questionnaires.

  3. Passport: add a copy from the passport for every person that needs to be insured.

8.Eligibility & Exclusions

  • Age limit: Enrollment is possible for any expatriate under the age of 65.
  • Countries not eligible for cover: Afghanistan, Iran, Morocco, Russia, North Korea, South Sudan, Switzerland, Syria, United Arab Emirates, United States of America. 
  • Cover outside the chosen geographical area (apart from Iran, North Korea, Syria and Afghanistan): during a trip lasting less than seven (7) weeks, solely for expenses incurred as a result of an accident or illness of an emergency nature. 
  • Expenses as a result of civil or foreign war, riots, insurrections, strikes, acts of piracy and sabotage, voluntary participation in brawls, popular movements or acts of terrorism, wherever the events take place and regardless of the protagonists (except in cases of self-defence) are excluded from cover. Members taken unawares by the occurrence of such events must leave the place of hostilities as soon as possible. Their cover remains in effect for a maximum of fourteen (14) days from the date of the outbreak of hostilities until the insured's return to his/her country of nationality or usual residence prior to expatriation;
  • Nationality: Available to all nationalities living outside their home country.
  • Local nationality: You can insure a local partner or children but only if the applicant (‘insurance-taker’) has non-local nationality. Examples: 
    • A Dutch applicant with a Thai partner living in Thailand: accepted. 
    • An UK applicant with a Dutch partner living in the UK: not accepted
  • Duration: Designed for long-term stays (more than 12 months). No cancellation possible during the first year, after the first year cancellation is possible with a 1 month notice.
  • Withdrawal after purchase of the insurance: theoretically possible within 14 calendar days of its signature, but very strict rules apply - see conditions. You do however have the right to cancel the application without giving a reason as long as the insurance has not yet started. Once the insurance has started the minimum duration is 12 months / one year.  
  • The consequences of an accident while practising an extreme sport or a risk sport are excluded from cover. These risk sports are: Ballooning, Alpinism, Apnea diving, Martial arts (except judo) and combat sports, Bobsleighing, Canyoning, Ice yachting, Nordic combined, Hang-gliding, Climbing (except on artificial support with safety system), Horse-riding (except dressage and walking), Ice hockey, Jet skiing, Speed skiing, Kitesurfing, Snowmobiling, Whitewater swimming, Parasailing, Skydiving, Paragliding, Paraskiing, Scuba diving, Cave diving, Diving, Polo, Rafting, Rallies and expeditions, Freestyle skiing, Ski bob, Heli-skiing, Water skiing, Skiathlon, Snow cross, Snow kiting, Ski jumping, Caving, Automobile sports, Motorcycle sports, Trekking and hiking at altitudes above 3,500 metres, ULM, Rock climbing, Navigation at sea more than 20 nautical miles from the coast.
